Probably the first on 8-bit Commodore machines, but I've seen it before in an Amstrad demo. Probably the reason it hasn't been done before is because it was a challenge to get the additivity out of just the black+3 shades without making everything white. On Amstrad, this can be solved more easily with the 16 color palette.

Took a little effort to find and compile an emulator that could run this, but it was worth it. Great stuff, especially the glenzkefrensbars - which I think I've seen on C64 before, but not this nice.
Great demo (should have ranked higher really) and unlike so many others actually has a final version (finally!).
A final version of the demo was made, which is much more polished and two screens longer.
